Kao i u mnogim drugim Urisovim knjigama, priča je uglavnom ispričana sa stajališta novinara; u ovom slučaju, američko-talijanski novinar, Christopher de Monti, koji je dodijeljen Varšavi nakon izvještavanja o Španjolskom građanskom ratu. Iako je trebao biti nepristrasan i neutralan promatrač, on se susreće i zbližava i s nacističkom hijerarhijom i sa Židovima u Varšavi. Ima strastvenu aferu sa suprugom jednog od vođa židovske zajednice, a bavi se i prostitutkama koje mu osiguravaju nacisti.

Dok je geto okružen i pretvoren u ruševine, on se pridružuje hrabrim braniteljima. On je jedan od rijetkih preživjelih i uspijeva pobjeći s mladom ženom Gabrielom Rak, koja je trudna s djetetom jednog od branitelja, Andreja Androfskog, bivšeg časnika poljske vojske.

Ime "Mila 18" preuzeto je od bunkera stožera židovskih otporaša ispod zgrade u ulici Mila 18.
