Kao i u mnogim drugim Urisovim knjigama, priča je uglavnom ispričana sa stanovišta novinskog čoveka; u ovom slučaju, američko-italijanski novinar Kristofer de Monti, koji je raspoređen u Varšavu nakon što je izveštavao o Španskom građanskom ratu. Iako je trebalo da bude nepristrasan i neutralan posmatrač, on se susreće i zbližava i sa nacističkom hijerarhijom i sa Jevrejima iz Varšave. Ima strastvenu aferu sa ženom jednog od vođa jevrejske zajednice, a takođe se bavi i prostitutkama koje su obezbedili nacisti.

Dok je geto opkoljen i pretvoren u ruševine, on se ubacuje u svoju partiju sa hrabrim braniocima. On je jedan od retkih preživelih i uspeva da pobegne sa mladom ženom Gabrijelom Rak, koja je trudna sa detetom jednog od branilaca Andreja Androfskog, bivšeg oficira poljske vojske.

Naziv "Mila 18" je preuzet od bunkera štaba jevrejskih boraca otpora ispod zgrade u ulici Miła 18.
