The book is supplemented by a DVD with the documentary film "Sicko", directed by Michael Moore, which shows the disastrous consequences of the American, completely privatized and primarily profit-oriented healthcare system.
Instead of privatization, Gorjanski advocates for the renewal and professional management of the health system, i.e. a set of health units organized and connected for the best possible achievement of the basic task, improving the health of the population and preventing impoverishment during illness. In his book "Renewal of Health", Gorjanski analyzes various forms of payment for the health system, from Bismarck's model of payment from employee salaries, through state payment from taxes, to private payment in the free market model, then various forms of payment for work in health care, such as payment of doctors according to service (which is the most widespread method of payment), according to the case, according to the number of patients, the well-known system of capitation, i.e. payment by regular salary, but also the negative impact of money on the efficiency of intellectual work. The author especially warns of possible conflicts of aspirations of the state, which would like to have the healthiest population, the most efficient health system, the most satisfied doctors and other medical staff, but also satisfied patients, and the full application of the medical code and the omnipresence of the rule of law. These aspirations almost completely coincide with the aspirations of doctors, who as a rule do not worry about the costs of treatment or at least put them on the back burner, as well as with the aspirations of patients, but they are the opposite of the aspirations of for-profit institutions. The source of income for for-profit institutions in healthcare is the patient, because there is no profit from healthy citizens. It could be said that the aspirations of for-profit institutions are the opposite of the aspirations of the state, doctors and patients, that is, they need the sickest population possible, the slowest recovery possible, the highest costs of treatment, the existence of interest groups and lobbies, and the absence of the principle of a legal state.
