The plot is set at the time of Italian unification and includes several patriotic themes. It was released by Treves on October 18, 1886, the first day of school in Italy, and was an immediate success.

Exploring social issues such as poverty, Heart shows the influence of leftist ideologies on the work of De Amicis (he later joined the Italian Socialist Party). This is why the book has remained influential (and an integral part of many textbooks) in the countries of the Eastern Bloc. On the other hand, the strong evocation of Italian nationalism and patriotism in the book made it very welcome in fascist Italy.
